Case Study: Integrative Management of Hypertension and Pre-Diabetes Patient Treatment

This is a comprehensive case study, I recently worked with which highlights the successful treatment of a patient of mine who had high blood pressure and pre-diabetes and disrupted sleep.

  • Name: Michael (referred to as such for privacy considerations)

  • Age: 59

  • Weight: 89kg

  • Height: 178cm


Background:


Michael, a 59-year-old male, presented to my functional nutrition practice with a history of hypertension and pre-diabetes. His symptoms included persistent high blood pressure, elevated fasting glucose levels, and disrupted sleep patterns, suggestive of Delayed Sleep Phase Disorder (DSPD). He was on antihypertensive medication, and his glucose levels were steadily progressing towards a diabetic range. His symptoms were tiredeness during the day, relying heavily on coffee and feeling extremely stressed all the time.


Assessment:


Upon initial evaluation, it was evident that a comprehensive approach was required to address his health challenges. Key assessments and findings included:

  • Nutrient Deficiency Analysis: Extensive blood work revealed several nutrient deficiencies, including vitamin D, magnesium, potassium, calcium, fatty acids and some others. These deficiencies are known to influence blood pressure regulation and metabolic health.

  • Advanced Lab Testing: Advanced cardiovascular and metabolic assessments were performed, including lipid profile, HbA1c, insulin resistance assessment, hormones and inflammatory markers.

  • Sleep Evaluation: A comprehensive sleep analysis was conducted to identify the presence of Delayed Sleep Phase Disorder. Melatonin onset measurements and full neurotransmitters analysis were performed to understand his sleep patterns and challenges.


Treatment Plan:


Collaboration with his supportive GP was integral to the success of the treatment plan. The approach focused on the following key interventions:

  • Food & Nutrient Supplementation

  • Medication Adjustment

  • Sleep Optimization Techniques

  • Education and Lifestyle Modification

Progress and Outcome:


Over the course of several months, his progress was closely monitored through regular check-ups and lab tests. The following positive outcomes were observed:

  • Blood pressure gradually normalized, allowing for the discontinuation of antihypertensive medication.

  • Fasting glucose levels returned to within normal range, preventing the progression to diabetes.

  • He reported improved sleep quality, reduced daytime sleepiness, and normal betime again.

  • Feeling more energy during the day and less stressed.


Conclusion:


This case study illustrates the successful management of hypertension and pre-diabetes in a patient through an integrative medicine approach. By addressing nutrient deficiencies, optimizing sleep aligned with circadian rhythm, and collaborating closely with a supportive GP, we achieved significant health improvements.
